Service Description

Therapy With Hart provides professional psychotherapy services to adults (age 16 and up) in Ontario through virtual sessions and in-person sessions in Wallaceburg, Ontario. Services include individual therapy, grief counselling, and support for life transitions, anxiety, depression, and trauma. I am a Registered Psychotherapist (Qualifying) with the College of Registered Psychotherapists of Ontario (CRPO). Psychotherapy services are not medical services and do not include diagnosis, prescription of medication, or medical treatment.

Therapist-Client Relationship

The therapeutic relationship is a professional relationship focused on your mental health and well-being. This relationship is built on trust, confidentiality, and mutual respect. The relationship is not a social, friendship, or romantic relationship. Professional boundaries are maintained at all times to ensure ethical and effective treatment. The therapist-client relationship begins at the first session and continues through termination or until either party chooses to end the relationship.

Scope of Services

Psychotherapy services provided include assessment, treatment planning, and therapeutic interventions using evidence-based approaches. Services are limited to psychotherapy and do not include medical diagnosis, medication management, crisis intervention outside of scheduled sessions, or emergency services. If your needs fall outside my scope of practice, I will provide appropriate referrals to qualified professionals. Virtual therapy is provided to clients residing in Ontario in accordance with CRPO regulations.

Informed Consent

Before beginning therapy, you will receive and review informed consent documents outlining the nature of services, risks, benefits, alternatives, confidentiality limits, fees, cancellation policies, and your rights. Your signature or verbal agreement indicates understanding and consent to these terms. Consent is ongoing and can be withdrawn at any time. You have the right to ask questions at any time about your treatment. I will document your consent in your clinical record.

Limits to Confidentiality

Confidentiality is fundamental to therapy, but there are legal and ethical exceptions where disclosure may be required without your consent: imminent risk of harm to yourself or others; suspected or disclosed child abuse or neglect; vulnerability of children or adults unable to protect themselves; court orders or subpoenas; professional consultation with colleagues; and disclosure to regulatory bodies as required by CRPO standards. These limits will be discussed in detail during your first session.

Emergency Protocols

Psychotherapy services are not appropriate for emergencies. If you are experiencing a mental health emergency or crisis, please call 911, go to your nearest emergency room, or call/text 988 Suicide Crisis Helpline, or another crisis hotline such as the Distress Centre (416-408-4357 in Toronto, or check your local crisis line). I do not provide 24/7 emergency services or crisis intervention. For non-urgent concerns between sessions, please email me and I will respond during business hours. If you are in immediate danger, please seek emergency help.

Cancellation Policy

I require 24 hours notice for session cancellations. Late cancellations or missed sessions will be subject to a 50% session fee. If you cancel with less than 24 hours notice due to a genuine emergency, the fee may be waived at my discretion. Repeated late cancellations or no-shows may result in termination of services. I will make every effort to accommodate schedule changes when possible with adequate notice.

Payment Terms

Individual therapy sessions are $140 per 50-minute session. Payment is due at the time of service and can be made by credit card through Jane App or by e-transfer. Receipts are provided for insurance reimbursement purposes. I do not provide direct billing to insurance companies. Outstanding balances must be paid within 30 days. Non-payment may result in termination of services. Sliding scale options may be available—please inquire if needed.

Liability Limitations

While I strive to provide effective professional services, I cannot guarantee specific outcomes or results from therapy. Therapy involves exploring difficult emotions and experiences, which may temporarily increase distress. You are responsible for your participation and engagement in the therapeutic process. I am not liable for decisions you make based on our discussions. My liability is limited to professional negligence and is subject to the limits of my professional liability insurance.

Termination of Services

Either you or I may terminate the therapeutic relationship at any time. I recommend a termination session to review progress and discuss next steps. I may terminate services if: therapy is no longer beneficial; your needs are outside my scope of practice; there are repeated boundary violations; payment is not received; or I am unable to provide services due to illness or other circumstances. I will provide appropriate referrals when terminating services. You may also terminate at any time without penalty.

Complaint Process

If you have concerns about the services provided, I encourage you to discuss them with me directly so we can address them together. If concerns cannot be resolved, you may file a complaint with the College of Registered Psychotherapists of Ontario (CRPO) at crpo.co or 1-855-427-6276. The CRPO has a formal complaint process and can investigate professional conduct matters. You also have the right to file a complaint with the Ontario Human Rights Commission if you believe your human rights have been violated.
